When you join one of our contracted health plans, you become our patient. 

Vermont Managed Care has brought together one of the most comprehensive provider networks in the state to provide you with quality and timely care encompassing the full range of specialties, and we keep on growing.

We believe in Primary Care.  The philosophy of a central care provider, or "Primary Care Provider" (PCP), is one of the keys to keeping you healthy.  Your Primary Care Provider will coordinate and manage all aspects of your care.  Should your particular health needs require it, our Care Coordinators will help your providers and family navigate and manage complex care plans.  While some health plan products don't require a PCP, we believe that PCP involvement enhances continuity in your healthcare.

Patient relationships with their providers is invaluable, and we want to do everything in our power to protect that.  In our view, education is key.  We want you and your provider to know as much as possible about the current health care system in Vermont and your individual health plan. That's why we've developed this web site as an on-line resource.
