As a provider driven PHO network, The VMC Care Management Department allows care coordination and utilization management to remain under the control of the network.  Through delegation agreements with our contracted payers, and under the stewardship of The Care Management Committee of the Board of Directors, The Medical Director, Dr. James Duncan, and The Associate Medical Director, Dr. Susan Shane, The Care Management Department coordinates, manages, and often facilitates needed care for over 40,000 covered lives throughout Northwestern Vermont. A dedicated team of Registered Nurses, all Certified Case Managers, will be your initial point of contact.

While case management involves working with Patients, Providers and Payers, the majority of our work lies with Patients and Providers.  Much of this work encompasses assessment, planning, facilitation, coordination, and evaluation.  All available resources are investigated and applied to handle any medical challenge that may present itself.
